Advance Directives /Advance Care Planning

Description: This clinic will focus on advance directives, their purpose, why we need them, and how they are enacted. Topics may include hospice services. A booklet containing advance care suggestions and an advance directive form will be made available.

Designed for: Anyone who wishes to complete their own advance directives and would like specific instruction on completing these forms.
